Patient Services

Our Patient Education Programs give you the tools to manage your health in a way that works for you. Whether you’re learning to prevent diabetes, manage a chronic condition, lower your blood pressure, or build strength and balance, our programs are designed for real life. We offer FREE classes and programs to help you make meaningful lifestyle changes at your own pace.

Many programs are available to older adults, and some are open to families and children as well. You can join through a provider referral, by self-enrolling, or after receiving a personalized invitation through MyChart.

Important Notes:

  • Start dates are tentative and require at least 10–15 participants per group.
  • All sessions are held in U.S. Central Time (GMT -6).
