Is Your Provider Separating from UTMB?

Thank you for choosing UTMB Health for your health care needs. We remain dedicated to expanding access to high-quality primary and specialty care across the communities we serve, even as we experience changes in faculty. Whether your provider is retiring, changing roles, or seeking career advancement outside of our system, rest assured, we are prepared to continue to provide excellent care.

You can remain with your current UTMB Health clinic or transfer to another one. If you need assistance with finding a new provider, contact our Access Center or review options in our online directory. Remember, you may need to update your insurance provider with the name of a new primary care physician or other caregiver, regardless of whether that provider is at UTMB or elsewhere.

If you prefer to change your care to a provider outside of UTMB Health, you will need to access your medical records and transfer them to the new facility.

Accessing Your Medical Records

To obtain a copy of your medical records, complete the online Medical Record Release Form. Alternatively, you can request this form via email or fax, by contacting the Health Information Management Department at (409) 772-1965. For more information, visit our Medical Records page.
