UTMB Health is providing proof of COVID Testing and Vaccination status using Smart Health Cards

If you received your COVID-19 vaccinations or testing at UTMB Health, you can now generate digital verification of your status via MyChart.

Using MyChart, patients can generate unique QR codes that contain their legal name and birthdate and either their most recent COVID-19 test result, or details of the COVID-19 vaccinations* they have received. Patients can access and share these QR codes to show their COVID status in three ways:

  • Patients can view the QR codes in the COVID-19 activity in MyChart. The codes can be scanned right from the patient's mobile device by anyone who requires proof of vaccination or test results.
  • Patients can export information from the QR codes to another app, such as a digital health wallet, and share the information from that app. The patient can choose whether to export vaccinations, test results, or both—they're in control of how much information is shared.
  • Patients can also print their QR codes by printing the page on which they appear from the MyChart website.

Don’t use MyChart? A printed copy of your QR code can be provided to you at your next clinic visit.

Note: These QR codes use the open source VCI standard (vci.org). The requestor needs to be among the organizations that use/accept the VCI standard in order to verify your COVID-19 status.


*At this time, the Smart Health Card record may not contain information on your third or booster COVID-19 vaccination. Your full vaccination history on record at UTMB Health, including all of your COVID vaccinations, is available under the “Immunizations” tab in MyChart.
