If you are unable to keep an existing appointment at UTMB, please reschedule or cancel it, so that we may offer that time to another patient. 

To cancel or reschedule, you may 1) use MyChart; 2) call your clinic; or 3) complete the form below. 

On this form, all fields marked with an are required. Information provided is submitted in a secure environment. Once you have submitted your information, if you selected to reschedule, you will receive a call or email with your new appointment information within two business days.
