The role hormones play in women’s midlife weight gain

Once women reach midlife, many encounter a familiar mystery that begins in the closet. A favorite pair of jeans suddenly won't zip, shirts feel tighter, belts need another notch, and the number on the scale keeps creeping up, even though their daily routines haven't changed.

Why weight creeps up

For many women, the extra weight isn't simply the result of eating more or exercising less. According to Sura Alqaisi, MD, an endocrinologist and obesity medicine specialist at The University of Texas Medical Branch (UTMB Health), hormonal changes associated with midlife, shifts in metabolism, and other health factors all influence how the body stores fat.

The changes can feel sudden, but they often reflect a combination of biology and lifestyle factors rather than a lack of willpower.

“Weight gain in midlife results from a combination of aging, declining estrogen levels, changes in energy expenditure and body composition, lifestyle factors, and increased psychological stress,” Alqaisi said. “All of these factors can contribute to weight gain. And why does it all seem to go to the belly? That’s a question I hear very often in my practice.”

The answer can be frustrating.

“It's because you’re burning fewer calories while your body composition is changing,” she said.

As women age, they lose lean muscle mass, and their resting metabolic rate declines, meaning the same number of calories that once maintained their weight may gradually lead to weight gain.

Even when the scale doesn’t change dramatically, body composition often does. During the menopausal transition, women tend to gain more visceral and abdominal fat while losing lean muscle.

“There are many possible reasons why you’re eating the same but still gaining weight,” Alqaisi said. “Women go through significant hormonal changes throughout their lives, and each stage affects how the body stores and burns fat. Women with polycystic ovary syndrome (PCOS), those who gained excessive weight during pregnancy, and women with insulin resistance often face additional physiological challenges that make weight loss more difficult.”

Sleep also plays a more significant role than many women realize.

“Disturbed sleep and sleep apnea can promote weight gain in midlife,” Alqaisi said. “Stress, mood changes, hot flashes, night sweats, and anxiety often contribute to these sleep problems.”

Poor quality sleep disrupts cortisol regulation and alters hormones that control appetite, increasing hunger and making weight loss more difficult.

The hormones behind the changes

Hormonal changes during perimenopause and menopause add another layer of complexity. As estrogen and progesterone levels decline, appetite, sleep, physical activity, and fat distribution can all be affected.

“Estrogen plays an important role in appetite regulation by influencing hormones such as leptin and ghrelin,” Alqaisi said. “Progesterone has a mild sedative effect through GABA activity and helps with sleep. When these factors overlap, hormonal changes become an important contributor to weight gain.”

Lower estrogen levels are also associated with joint pain and loss of muscle mass, both of which can reduce physical activity.

Some weight gain during the menopausal transition may be difficult to avoid.

“Even with healthy lifestyle changes, some weight gain during the menopausal transition may not be entirely preventable, but it can be significantly reduced through healthy eating and regular exercise,” Alqaisi said. “For women who already have obesity before entering perimenopause, it can be even more difficult because excess abdominal fat promotes insulin resistance, and declining estrogen can make that insulin resistance worse.”

When weight gain signals something more

So how can women tell whether their weight gain is driven by hormones or something else?

Alqaisi recommends a medical evaluation if weight gain is rapid, unexplained, or accompanied by other concerning symptoms.

“If a woman develops persistent weight gain despite healthy lifestyle changes, especially if it is accompanied by fatigue, cold intolerance, constipation, easy bruising, muscle weakness, purple stretch marks, or new menstrual irregularities, it is important to evaluate for secondary causes of weight gain,” Alqaisi said.

“Depending on the clinical presentation, we may screen for conditions such as hypothyroidism, Cushing syndrome, and other endocrine disorders. We also evaluate for obesity-related conditions, including diabetes, fatty liver disease, high cholesterol, high blood pressure, and metabolic syndrome.”

Physicians also review menstrual history, sleep quality, and medications that can contribute to weight gain.

“We ask about changes in menstrual cycles, including very heavy or irregular periods, because they may provide important clues to underlying hormonal conditions,” she said. “A careful review of medications is equally important, as certain antidepressants, antipsychotics, steroids, and some diabetes medications can contribute to weight gain."

Alqaisi said she would also discuss smoking cessation with patients because, although quitting smoking has significant health benefits, modest weight gain afterward is common.

"Rather than discouraging smoking cessation, we focus on strategies to minimize weight gain through healthy nutrition, regular physical activity, and ongoing support," she explained. 

If testing rules out endocrine disorders, sleep disorders, medication-related causes, and other medical conditions, treatment focuses on sustainable lifestyle changes tailored to each woman's health, body composition, and long-term goals. When appropriate, anti-obesity medications or other evidence-based therapies may also be considered.

What actually helps

Successful treatment usually combines healthy eating, regular physical activity, and, when appropriate, medication.

“We start with an exercise prescription,” Alqaisi said. “We encourage women to gradually increase their daily step count and incorporate both aerobic and resistance training. We also discuss nutrition, focusing on adequate lean protein intake, reducing refined carbohydrates, and, when appropriate, meal-timing strategies that fit the individual's lifestyle and health goals.”

For some women, medications may also improve outcomes.

“For women who are candidates for both glucagon-like peptide-1 (GLP-1) receptor agonists and menopausal hormone therapy, addressing both obesity and menopausal symptoms may provide additional metabolic benefits,” Alqaisi said. “Although menopausal hormone therapy is not a weight-loss treatment by itself, it may help improve body composition, reduce visceral fat, improve insulin sensitivity, preserve lean muscle mass, improve sleep, and support bone health when appropriately prescribed.”

For many women, the number on the scale tells only part of the story. Rather than blaming themselves for unwanted pounds, Alqaisi encourages women to recognize that midlife weight gain is often the result of complex biological changes.

The hormonal shifts of midlife may be unavoidable, but understanding what drives those changes can help women make informed decisions about lifestyle changes, medical evaluation, and evidence-based treatment.

“Women shouldn’t blame themselves,” Alqaisi said. “Midlife weight gain is influenced by complex biological, hormonal, and lifestyle factors. Understanding what’s happening in the body is the first step toward developing an individualized treatment plan and achieving long-term success.”
