An easy and efficient screening featuring UTMB sports medicine doctors, orthopedists, family medicine physicians, cardiologists, pediatricians and pediatric specialists, physical therapists, and others. Only $25 per student.

Does your child or teenager need a pre-participation physical screening for his/her school, team, or summer activity? Start the season with our UTMB medical experts, at a special community outreach event to support local families, school districts, and athletic and music programs with pre-participation physicals for extracurricular activities.

Students should wear t-shirts and shorts, and bring school-specific forms. Walk-ins will be welcome (as time permits).

To save time at the screening, students should pre-fill their school forms (see list at right) with their names and general information before they arrive.

One of the required forms is a UTMB Health Consent Form. You may DOWNLOAD THE CONSENT FORM HERE.*

(*Parents: If you will not be at the screening with your student, you must send him/her with a completed and signed UTMB Health consent form, along with any signed school-required forms.) Please note: This sports pre-participation examination is not a substitute for the routine health maintenance care that should be provided by a personal physician.
